What if I have a question and I need to talk to the doctor?

If it is an emergency or you think you are going into labor, please call the office during business hours and your call will be triaged and directed to the medical assistant or office manager. If they feel that it is a serious matter, the doctor will be informed and may either choose to talk with you or have you come in to be evaluated.

After office hours or on weekends, if you feel it is an emergency or are having a serious matter, please contact us through our answering servic (800)-428-2269. If it is not an emergency, please contact during normal business hours.

How can I find out if my labs/test results are normal?

Our office has a general policy that if you are requesting the results of your tests, you may call and leave a message and the medical assistant will return your call in 1-2 business days. The doctor will always call you directly to discuss abnormal test results.

How can I request forms to be filled out or faxed to other providers?

Forms that need to be filled out by the doctor are subject to a $25 fee. Please allow 1-2 weeks for completion of all necessary forms.

Forms or charts that need to be faxed can be done after a medical release form is signed in the office.

Copies of charts for transfer of care are subject to a $35 fee and will require atleast 1 week's notice for preparation.

What is I have a question regarding my bill or copay?

It is generally the policy of AWHS that you enquire with your insurance carrier prior to your office visit. Any co pays or additional charges that are not covered by your insurance will be expected to be paid in full at the time of your office visit. Any further questions may be directed to our billing service.
